Out of Network:
Our private practice is considered "out of network" and does not accept any insurance plans/payments. Payments must be made directly from the patient in form of cash, check, debit, or credit card.
However, many insurance companies will reimburse a portion of your payment. Our office can provide a "superbill" which you may use to submit claims to your insurance company. Our office does not submit claims on the patient's behalf.
You also can use your HSA or FSA account to pay for our services.
If you are unable to keep your appointment, please provide notice within 2 business days, or you will be charged the entire fee for your missed appointment.
Our doctors do not fill any forms including but not limited to disability forms, legal forms, or FMLA.
